This notice explains how Generations of Hope handles information submitted through this website, including the consultation request form. It does not replace any program-specific form, participant agreement, clinical privacy notice, or other document provided when someone enrolls in a service.
The consultation request form asks for basic contact information, a preferred way to be reached, the kind of support you are looking for, availability, and an optional practical note. The form specifically asks visitors not to include health details, diagnoses, insurance numbers, or urgent information.
Generations of Hope uses the information in a consultation request to review the request, follow up in the selected way, help identify a practical next step, and support scheduling when appropriate. A first request is not a clinical assessment and should be kept focused on practical needs.
If a program includes group participation, other participants may hear what is shared. Generations of Hope asks participants to respect one another’s privacy and prohibits recording or sharing group information without permission, but cannot guarantee what another participant does after leaving a group.
